Khem Val quests not happening

I completed my class story and talked to all of my companions as much as I could, however I didn't get the choice between khem and Zash. I went with Khem to Tatooine but after that nothing.

I noticed that although I have every class through the storyline and every companion for every class through its conversation line there is one exception. Khem. I know I did all the conversations that were available on multiple characters but Khem still shows as the only one of the original story companions not have been maxed out in conversation path. So either it is bugged or, perhaps more likely, whatever you missed I also missed and more than once.
