NPC evading during combat

I have no way to exactly replicate, except that is only happening on Makeb and more noticeable during the staged weekly, but I barely do anything on Makeb expect the weekly quest.

 

Bug: Enemy NPC "Evade the player", by evade I mean as in the following example:

1. Force charge an enemy
   
2. Start the fight
   
3. Move through the NPC as I kill them
   
4. At no specific point during attacking one of the NPC an "Evade" pop up on the remain NPC -even when hitting the NPC-
   
5. After the evade, I must hit the NPC again to start the fight all over again.
   

I want to be clear at no point at all I am moving them away from their postion, I have noticed this bug on my Melee DPS character (a Marauder).

 

I can try and get a video of this, if and when it happens again.
