Has anyone managed to find the 5th codex entry? (1189, -1271) has some additional wreckage and is depicted in the codex image: http://www.swtor-spy.com/codex/rakghoul-pandemic-5-origins-of-the-outbreak/49712/ Still no idea how to get the entry.

Dont have exact coords on me. Urtagh is in the area walking around, Trapjaw is in his normal spot just infected now (lvl 30-ish world boss), and Zama is next to the Sarlacc pit (clicky wreckage next to pit if its there).

For anyone that stumbles upon this thread looking for the answer, the answer is yes! I wound up making it an you can indeed get an augment slot on the relic if you're lucky!

Does anyone know if it is possible to get a crit [Mastercraft] on the BoP Rakata relics that Artifice crafters can create? If I understand correctly, the item modifications cannot crit, but the offhands can. However, I was unsure about the epic relics as well. It would be amazing to get an augment slot on them if it were possible Thanks!
