I have obtained the entry. Though it was so long ago, I remember that the swtor-spy description which I believe is what you referenced there was misleading. I did climb up something and then down into a big pit where I killed colicoids to get the entry.

Your persons of note looks accurate for a Sith Warrior. The Denova codex I think you are referring to is a minecart halfway through the cave after Toth and Zorn. I see now, thanks to your ss, which PvP codex I am missing, leaving us with only 2 bugged.

Yes, you receive it upon completion. Currently, you have to complete all lead quests to receive this. However, before Thursday's patch Empire players could attain all the items for the race by completing a certain space mission.

I would love to update it with the other classes, however since I am not chasing entries with my alts it will be difficult to compile an accurate list for the other classes. I have no issues adding and crediting anything submitted here. At the moment, we only have the 1 entry you mentioned... and adding that one in any way would make it seem out of place. As for the extra tab it is REALLY skinny and at the very top of the list of categories. Again it has no name, but if you roll over it you'll see.

It is not bugged for BH/IA. As stated in the original post, this list is compiled from a BH codex. Which is why I have listed all the Korriban codex entries that a Warrior could get.
