where do you turn in the tokens from the hard mode flashpoints and what are the crystals used for?

hello i was trying to find an orange belt for my assassin as that is the only slot left to get an orange item for. the only problem is that i am unable to find one so i am asking if there is a fairly easy belt to get from and instance or other place? also where do you find the orange armor with the augment slot? thanks for your time.
