When repeating KotFE Chapter 2, at the point where you see your original companions, Qyzen Fess is naked (just wearing a pair of briefs). This, despite the fact that (1) he's geared in a nice armor set, and (2) he has a companion customization. Here's a picture showing what I mean. Note #1: The customization doesn't show up either.
