Basically shortly before battling the real Red Blade before leaving the planet with my Imperial Agent I managed to accept the quest completion reward before he started shooting. The one with the red carbine he gives ya before trying to kill ya. It showed up again after I killed him so I got two Red Carbines and two Skill Mod 1s. Not sure if that's supposed to happen.
