Playing KotFE on my main during the early access, I went through the process of flirting with Koth. To the best of my knowledge, I did every [flirt] option and am pretty sure I dd the "[Flirt] I need to talk to one of you". So I assumed that we were doing a relationship in the style of 1-50. Yesterday, I finished KotFE with an alt and completed the Romance with Lana. I saw a box pop up saying if I continued, I would begin a romance. I also noticed there was a notation on my companion pane that I was in a relationship with her. So I guess the bug is that I should be in a relationship with Koth, but am not.
