Darth Zash explodes and disappears at the end of "In the Halls of Traitors"

This has happened to me twice in the past two days, while playing two different female Sith Sorcerers on server Begeren Colony.

 

For the class mission "In the Halls of Traitors", the final cutscene is the conversation with Darth Zash where she tells you that she has given you your starship. Immediately after that cutscene ends, then Darth Zash explodes and disappears.

One of the latest instances had it happening to Lord Zash on Dromund Kaas, and then her character model disappearing (after a cutscene, not during.)
