Looks like this achievement is not bugged but simply very poorly designed. Ticket answer: My name is Meghan from Star Wars: The Old Republic Customer Service. Thanks for contacting Star Wars™: The Old Republic™ Customer Service to report the issue you encountered concerning the achievement "One for All". After investigation, I have confirmed that there is no known issue concerning the achievement. However, I have found that if the last blow to the Exarch is delivered by environmental damage or a companion, the achievement is lost and you won't qualify for it. As a healer you're screwed over then since you HAVE to take all the damage then in the meantime while you're ill-equipped for that and with relative low DPS.
