She was betrayed by a Sith Lord named "Scourge" when the Revan, the Exile, who's name was "Meetra Surik" and Scourge all attempted to kill the reigning Emperor. Scourge killed her with his light saber by running it through between her shoulder blades. She died before she hit the floor. Revan was captured. Scourge lied and said it was all a plot he was trying to foil. Scourge was rewarded. Revan was kept alive in stasis in a state between life and death where the Emperor could feed off him.
