Looking at a couple of logs from my assassin to my marauder and I saw that I only missed 1% of my attacks on my assassin. On my marauder I generally miss around 10% of my attacks. Both of them have a little over 100% melee accuracy. Off-hand attacks have a base 66% chance to hit (76% for Special Abilities, anything but strike) and benefit from Accuracy. So if you're sitting at 100% Melee Accuracy your off-hand attacks will have 76% Chance to hit, and 86% if they're special attacks.
