[Blood Hunt still not fixed, EA still worst company in the world]

My son and I just played this and when the husband and wife team killed us the first time, we decided to give it another try with some better tactics. We were at the point where the husband and wife were fighting us together. The husband was nearly dead with the wife at half life, then the husband punched me out of the instance. My health was just shy of full and I flew out of the instance and ended up behind the door, a few seconds later this happened to my son. Does anyone know what this is?
