My reply is you stood somewhere you shouldn't or an ability wasn't interrupted or you weren't healed. Simple fight awareness from yourself and others will fix it.

Your reply is that one of three VERY different things happened, with three very different causes, and three even more different solutions. Really? How do we know which one it is? What happens if it is a combination of them? How do we solve this without a combat log please?

I got the same mission, I think it's a prelude to the upcoming flashpoints in the next major patch that perhaps got accidentally added with the hotfix?
