Im a subscriber , Thank you Did you claim it from the Cartel Market? Did you, afterwards, equip all eight pieces (even just for a moment) on your character? Um, not all eight at once, obviously, but you have to equip both helmets (the breath mask and the full mask), one after the other, in order to unlock it in Collections for that character.
