FriarChuggs
After a wipe on Soa last night we returned to find about a third of the pieces of the Soa platform were missing, so when we entered phase 2 there weren't any places to safely jump down in a number of spots. This problem persisted through several more wipes. We attempted a reset of the instance, but the puzzle doors were closed after reset, and we could not re-initiate the puzzle encounter to open them. Anyone know how to avoid/fix this bug?
Navimygator
In our instance the doors just opened when you walk near them.
FriarChuggs
In our instance the doors just opened when you walk near them. I hope this is the case for us, we saw they were closed and didn't approach. /facepalm
Lostpenguins
I hope this is the case for us, we saw they were closed and didn't approach. /facepalm Yeah, if anyone in your group exits area after phase 1 drop-off it can cause the missing platform bug problem. You need to have everyone die and hit reset. It's a farther run back, but the doors do open.

Anyone know how to avoid/fix this bug? Reset the encounter after EVERY wipe. There is zero downside to doing this and a lot of bugs can be avoided from doing it. You can know the encounter is reset as those doors will be closed and Soa does his speech. Approach them and they automatically open. Hope that helps. VulcanLogic
This bug is still there and there's nothing in the patch notes about fixing it. Happened three times tonight even after hard resets. Pretty angry about it, as Bioware wasted an entire evening for us.
SkuzBukit
This bug is still there and there's nothing in the patch notes about fixing it. Happened three times tonight even after hard resets. Pretty angry about it, as Bioware wasted an entire evening for us. It does sometime get into this "Hard broken" state & refuses to reset (sometimes reset even cannot be clicked). The few times my guild encountered this what we had to do was exit the phase> reset instance if possible > leave Operation party > have a new Operation leader re-invite people. This "reformation"! usually worked, if that failed totally abandoning the quest, including weekly, then following the above steps helped & on 2 occasions absolutely nothing we tried would get rid of the bug & after trying to complete it one of those times with that bug in place, eventually got ourselves to the bottom of phase 3 in pretty bad shape only to find that he refused to drop his shield when a pillar was dropped on his head we called it a night & ended up leaving it until after a server maintenance. Hope some of that helps & GL beating the bugs & him. Edited April 6, 2012 by SkuzBukit

There is 1 downside, you've got to run further since the speeder doesn't drop you as close post reset.

Yeah, that is a giant slap in the face. We cut Thursday nights short due to work schedules, and this only made it more frustrating as we got fewer attempts. That, and the pylons also bugged out and we had to reset that and reclear trash. Not happy about it.
Valan
Missing platforms will happen if you call a wipe (or actually wipe) while doing the phase transition. To avoid it, we always called a wipe during an actual phase of the fight with a full platform, or if already triggered, we would tell whoever was left alive to do the platform phase and wait until soa appeared on the bottom floor before wiping. This has never caused the floor to bug out for us.
