Some planets have specific companion customization vendors (check the vendor hubs at the starting area for each planet) and there are also some on the CE and authenticator vendors. The ones I have seen have ranged in price from 12k-35k. They change skin color, hair style, tatoos, etc. You can preview them by Ctrl clicking.
