Ah, what's going on with the CS team? I added a ticket to have a character restored, provided all the details, sever/name/class etc.. and they came back with this and closed my ticket:

When this happens, we recommend that you try resetting the mission. This usually clears up the problem.

You can also report the bug to the development team by using the /bug command in-game.
