I'll add to your thread. Not really a bug, but kind of annoying. Since you can play huttball without actual groups, the 45 second self-destruct timer needs to be removed if you play it 1 v 1.
On my commando, waiting for the balloon on Tat. Get jumped by a group. I rez as the balloon gets there, the group jumps on and I follow while still in the rez stealth. I get to the center of the balloon and wait for the stealth to drop then hit my knock back. All 4 in the group go flying to their death while I go on to get my datacrons.
In a word. Launch.
The game was mismanaged from the beginning. Choosing a half finished engine and sticking it with poor design decisions & to many servers.
Keeping the 2 factions completely separate with no interaction.
Focusing on attacking 3rd factions instead of utilizing the primary conflict of Star Wars.
Yes, many will come back, but many won't. Each event (summer, school, holiday's) will bleed many people off the game and a few will never return. These few keep adding up and will eventually lead to no one logging into the game.

The problem isn't the players, even with them being toxic the problem is the game design. I can't comment on Tera, but I can on SWTOR. It was not designed with OWPvP in mind and for it to be successful the game designers need to plan for it from the beginning. Just a couple of things that should be in every triple A MMO:
There should always be 2 spawn locations available, giving players a way out of being camped. There needs to be a penalty when you kill players that are lower level than you. Ignore lists should not have a size limit and they should be legacy wide. A responsive customer service. You should be able to report something and have a response within 30 minutes.
It's not about volume of people that you kill. It's about your motivation for the killing. If you do it for the greater good and in self defense you can kill millions. If you do it because the green yoda wannabe is annoying and just needs to die, then 1 is enough.
Mine is people that use knock backs after I gather a group together so that I can use my AOE and preventing the satisfaction that comes from Mortar Volley or Death from Above.
Except no one had actually tested hindered and were jumping to conclusions.
Personally, I still think something similar to hindered needs to be put in place. However, instead of blocking those abilities from being used, just cause the ball to be dropped when the ability is used.
I haven't researched prebuilt systems, and my new build was almost $3k. So, I'm not going to give specific recommendations. But, I have answered a couple of your questions.
Different games have different levels of maintenance mode. Generally, larger publishers will continue to have some development continue till the game is shut down. Smaller publishers are more likely to stop development and move on to something else while keeping the servers up in a hopes of funding the something new from the existing game population. Larger publishers don't need this income to fund new games.
Mine from launch started acting up, not always responding to pressing the button. So, I went ahead and switched to the google authenticator. I've been happy with it. I do recommend to make the switch before the battery does die and add on extra steps for making the change.
I recommend to check out Starfighter Inc. Several of the developers actually worked on the X-Wing series of games.
It just hit Alpha, so is still a little ways out. But looks very promising, especially their VR support.
